Vaccines- Are required/alternative vaccine schedule permitted.

Office hours- Monday through Thursday 8:30-4:30, Friday 8:30-
12:00. A pediatric nurse practitioner is available after hours and
on weekends to triage your child’s illness.

Copayments are due at time of service. This is a contract
between you and the insurance carrier and must be collected.

No Show policy- if you are 15 minutes late to your scheduled
appointment, you will be marked as a no show and will be asked
to reschedule. Since we know life happens, three no shows will
get a letter of dismissal.

Form completion policy- we require 7-10 business days for
completion of any forms

Prescriptions - ADHD medications are controlled by the DEA and
these children must be seen every 3 months. Please request any
refills for other medications at least a week before the child runs
out of medications.
